Navigator Enrollment Services

  • The HWCLI Healthcare Access Team helps our clients find a health insurance plan that best fits their medical and financial needs. We have an outstanding health care team of certified navigators that help individuals and families enroll for free or low-cost health insurance plans through the NY State of Health Marketplace. Open Enrollment Period will remain open for all Marketplace programs for the duration of the federal public health emergency. Enroll with one of our navigators today!

    Our services include:

    • Health insurance enrollment assistance through the NYS of Health Marketplace
    • Determining if you are eligible for free health insurance or financial help for private health plans
    • Helping you compare health plan options and prices that best fit your medical and financial needs
    • Helping you understand health insurance terms and policies
    • Advocating on your behalf with any issues you have with the NYS of Health Marketplace

    Our team can help you with the above in English, Spanish and Haitian Creole.

Community Health Advocacy

  • As Community Health Advocates, we provide free, post-health insurance enrollment guidance, helping Long Islanders navigate through our complex health care system. We provide health care advocacy, educational resources on health insurance, and help clients use their health coverage.

    HWCLI’s Community Health Advocates can help with various healthcare matters, including:

    • Understanding Medicare and supplemental plans
    • Doctor and specialist referrals
    • Issues with your health plan
    • Billing issue resolution
    • Prescription assistance programs
    • Non-MAGI Medicaid with Nassau and Suffolk Department of Social Services
    • Information on accessing Long Island’s  Federally Qualified Health Centers (FQHCs)
